Every effort is being taken to keep down the cost of good dental care. Reducing billing costs will help minimize necessary increases in fees. For this reason, payment is requested at each visit unless other arrangements have been made in advance.
Many families are fortunate to have dental insurance. There are differences in the benefits provided by various insurance plans. To help you understand your plan, please read the insurance booklet supplied by your employer.
As a courtesy to you, our office will complete, submit and follow-up on initial insurance claims for you. However, complete and accurate information about your particular plan is essential for us to submit for you. Your insurance card and/or form issued by your employer should be brought to the first visit. Please remember any insurance policy is a contract between you and your insurance company. The parent who brings the child is ultimately responsible for the payments. Remember, too, that no insurance company attempts to cover all dental costs, not even companies that have ‘full-pay” plans. It is your responsibility to pay any deductible, co-insurance or any balance not paid by your insurance. These balances are due and payable at the time of each treatment visit.
